Many students ask about the ability to complete clinical hour requirements at a hospital or EMS agency close to them. This is possible, depending on the location and willingness of the site. For this process, we ask that any student that wishes to use a site not already in PERCOMOnline's clinical network to do the initial contact at the site themselves. If the site is willing, the student will then let PERCOMOnline know using the method below.


STEP 1.

To add a new clinical site, first check the Clinical DISCLAIMER FAQ to ensure your state is available for clinical sites. 


If it is not, either we do not have an Asst. Medical Director for the state or the state does not approve of out-of-state programs. To learn more about why your state is not on the list, please send a support ticket.


STEP 2.

Download the New Site Request Form and MD Acknowledgement forms found at the bottom of this article. Please either print it out on paper, or saved digitally in your smart device.


STEP 3. 

Carry your digital/printed copy of the New Site Request and MD Acknowledgement Forms with you to the medical care provider you wish to add to PERCOMOnline's clinical network. Talk to the person in charge of Clinical Agreements at the site, and if they are in agreement have them complete the form or provide you with the information for completing the form. 


STEP 4.

Email the completed form to our Program Director, Mrs. Dinsmore, at programdirector@percomonline.com.

STEP 5. 

Allow time to process. After you submit the request form, our Program Director will review and contact the site directly with more.
